An Australian National Health and Medical Research Council report on road tunnel air quality has found that the M5 East tunnel in Sydney, Australia is amongst the most polluted tunnels in the world. This has led to concerns that it could pose health risks to commuters and people who live nearby, aggravating asthma, respiratory infection and possibly promoting cancers. The National Health and Medical Research Council said the 4 km-long M5 East tunnel was particularly congested, carrying 100,000 vehicles/day, and particularly long for an urban tunnel.

The report found evidence suggesting short-term exposure to traffic pollutants, particularly nitrogen dioxide and particulate matter posed health risks and the M5 East tunnel, when compared internationally, has among the highest levels of these toxins. Average nitrogen dioxide concentrations in a range of tunnels around the world were mostly in the range of 50-150 parts per billion but the M5 East recorded levels of more than 200. However, a lot of work has been done to improve the air quality, including installation of 12 new jet fan systems, a gas monitoring system and a video system to detect smoke from trucks.

In the longer term, a feasibility study is underway for the duplication of the M5 East tunnel.
